REMINDER!  Immunization and Health Examination Requirements

Public Health Law Section 2164 

Dear Parents, Guardians and Caretakers,

It has been over a full year since the repeal of non-medical exemptions to school immunization requirements in New York State. The New York State Department of Health (NYSDOH) has reminded us that schools are expected to remain vigilant in fully complying with Public Health Law Section 2164, in order to protect children in this state against all vaccine-preventable diseases. 

This means that NYSDOH expects any students who missed doses of vaccines due to COVID-19 to receive any overdue doses within 14 days after school beginsregardless of whether students attend in person or virtually.  This may only be extended to 30 days for students transferring to the district from out-of-state or from another country. 

School began on September 14, which means students have until Monday September 28 to show proof of required immunizations.  Students will not be permitted to attend school (in person or virtual) as of September 29, 2020, if they are missing any required immunizations.   

Nurses have been making phone calls and sending letters home to notify families of any missing immunizations since the Spring.  

Ellis has a walk-in clinic available.  

Ellis Primary Care Plus, located at 600 McClellan Street, is offering a walk-in immunization clinic from 3:30-6:30 p.m. this week Monday, September 21 through Thursday, September 24.  

No appointment is needed. Please bring your insurance card. If your child is a non-Ellis Primary Care patient, bring immunization records/forms provided by the school or contact the school nurse so she can provide them directly to Ellis.

New York State Law also requires a Health Examination for new entrants and students in grades Pre-K or K, 1, 3, 5, 7, 9, & 11.
Completed Immunization and Health Exam forms may be mailed or faxed to the attention of the School Nurse at your student’s school.
