Schenectady County Public Health Services is working with the Schenectady City School District to offer the H1N1 flu vaccine to school age children within the district.

FREE CLINIC

Your child can get vaccinated on
Saturday, December 5, 2009
at Mont Pleasant Middle School
(in the Gymnasium).

Please enter through the gymnasium doors in the back of the building, near the park.

You must make an appointment.

Steps you need to take:

Be sure to read the above information statements.

Fill in the Patient Information and the Emergency Contact.  Please answer questions 1-14 on the Consent Form (which was sent home with your child).

Bring the completed Consent Form with you when you come for your child's appointment.

A separate form must be completed for each child.

Your child will receive either the injectable or the nasal spray depending on the results of the medical screening and vaccine availability.

Please take advantage of the opportunity to protect your child from this illness.

Making an Appointment

If your child's last name begins with the letters A through M
please call 631-0840 between 8:30 a.m. and 12:30 p.m., Monday - Friday.

If your child's last name begins with the letters N through Z
please call 631-0840 between 12:30 - 4:30 p.m., Monday - Friday.

This clinic is limited to school age children.  If you have child between six months and school age, please check with your child's primary care provider for vaccine availability or call Schenectady County Public Health Services (SCPHS) to schedule an appointment at one of the on-site clinics.

Booster

If you child is less than 10 year of age and has had the 1st dose of H1N1 vaccine at least 28 days prior to the clinic, you can make an appointment for the booster for this clinic.

Weather

If there is inclement weather on the day of the clinic, please check your local radio and TV stations for a change in schedule.
