Any student trying out for a  sport must show proof that he or she has had a physical and submit  the following completed paperwork by the due date.

 

Emergency Card Health History/Update Form Physician's Form (Must be completed and signed by physician) Athletic Code of Conduct Recent Health Update Form If you turned your sports packet in before August 20, you must bring this form completed to the first day of practice.
